INTERACT™ Center of Excellence
Organizations discover improved clinical outcomes through the INTERACT™ QIP training and resources. Our certification for individual facilities, networks and health systems, INTERACT™ Center of Excellence, will aid your focus on providing quality care and managing acute changes in patient conditions. INTERACT™ Center of Excellence designation highlights the commitment to an intensive focus on the following:
- Delivering leadership and direction for embracing full INTERACT QIP implementation.
- Identifying best practice approaches for utilization of the INTERACT QIP.
- Maintaining INTERACT™ Certified Champions.
- Monitoring data and clinical outcomes to identify areas for performance improvement.
- Providing ongoing training and support for your care delivery team.
INTERACT™ Center Of Excellence Designation:
- Center of Excellence agreement criteria is reviewed and signed between the client organization and Pathway Health (Certifying Entity)
- Certification is valid for two years
- Must have two INTERACT™ Certified 4.0 Champion co-champions (Primary and back-up) at all times
- All Facility Managers and Leaders – required to attend a one-day Leadership training on INTERACT® and how to support the program within the organization
- Onsite training for all levels of the organization on INTERACT™ QIP, P&P as well as their respective roles and responsibilities. INTERACT® is included in the orientation program for all staff and annual staff development
- All Clinicians must attend INTERACT® online orientation
- Medical Director
- MD, DO, NP, PA who provide regular care to multiple patients in the entity
- Medical Director must sign the COE agreement
- Medical team orientation on INTERACT® and expectations from the organization
- Approval of the program from the facility Medical Director
- Use policy and procedures for implementation of INTERACT™ QIP
- Data and metrics – aligned with INTERACT® program
- Minimum requirements include:
- Structured RCA on hospital transfers including, tracking hospitalizations, Emergency Department visits and observation stays
- Utilize INTERACT® tracking tool or approved comparable tracking tool/system
- Usage of INTERACT® within client EHR
- COE market place data
- COE will provide referral entity data including
- Primary and secondary referral sources and demographics
- PCPs engaged with the client facility and demographics
- INTERACT™ Certified Champion and organization will have access to INTERACT® Master Trainers and information portal to assist with questions, implementation strategies and updates as indicated
- Follow Up:
- The facility will set up three quarterly web-based reviews and a year one review on year one and complete two bi-annual reviews on year two to include:
- Program Implementation
- Data/Analytics Review
- Performance Improvement Opportunities
- Action Plan developed and provided to facility, based on review.
